You’ve developed a plan (Stage 1), standardized processes (Stage 2), and analyzed data (Stage 3). So, what’s left? Well, now you need to optimize your plan with the insights you’ve gained from the journey so far. Optimizing your plan means taking all the gathered data and using it to close the gaps and make the necessary improvements before implementation. With the completion of your analysis in Stage 3, you are ready and able to use your learnings to update policies and procedures and confirm your implementation plan.
This stage is particularly important as it is the final alignment between all disciplines and services provided in the home. It is also when your transition processes need to be confirmed as this is the final preparation stage before operationalizing all changes across your organization.
Here are a few key strategies to keep in mind during this stage:
- Review Your Change Management Plan. It is beneficial to re-examine and reinforce the plan you developed in stage one using the data and insights collected in previous stages. Remember, keep an eye out for gaps and create a plan to optimize going forward.
- Implement Quality Assurance (QA) and Performance Improvement (PI) Programs to Close Identified Gaps. Did you know that QAPI is mentioned 212 times in the final rule for the requirements of participation? Using a QAPI solves two problems at once, making it easier for you to build efficiencies into your plan.
- Examine Additional Revenue Options. Always be on the lookout for specialized services provisions to close the gap between occupancy and revenue.
- Determine Your Resource Needs. Now that you have gathered all your data and reviewed it for gaps, you are in the perfect situation to evaluate the resources you’ll need to successfully transition to PDPM.
