Patient Transition Study: Technology Use During Transitions of Care

PointClickCare’s 2019 Patient Transition Study was conducted in partnership with independent research firm Definitive Healthcare. Participants, including c-suite executives from acute and post-acute care facilities, provided input on technology use trends during transitions of care.

Introduction

While electronic health record (EHR) platforms are nearly ubiquitous in hospitals and skilled nursing facilities, many healthcare professionals struggle with — or have reservations about — sharing data with their healthcare partners. PointClickCare’s 2019 Patient Transition Study was conducted in partnership with independent research firm Definitive Healthcare1. Participants in the blinded survey, including c-suite executives from acute and post-acute care facilities, provided input on data sharing, concerns about interoperability, and other pressing pain points
in care delivery and coordination.
