CUSTOMER PROFILE - ACUTE TriHealth is a unified health system in Cincinnati, Ohio, that was created when Bethesda and Good Samaritan Hospital joined together in 1995. Through these two hospitals and more than 130 sites of care, TriHealth offers a wide range of clinical, educational, and preventive programs. Staff members at TriHealth previously had to search through paper records to find patient information. The hospital would send physical continuity-of-care forms forms to the skilled nursing facility to inform them about patient needs, which was then manually entered into the EHR. This common method led to a gaps in care, and staff wanted a way to make transitions seamless, automatic, and secure. Partnering with PointClickCare TriHealth implemented Harmony by PointClickCare, so staff no longer have to search through paper records to find the information they need, and families are able to make the best transition decisions for their loved ones.
