Achieving Success in the World of Standardized Healthcare

Value-based, standardized care is the new focus in the healthcare industry, and providers are facing more pressure to deliver the best possible care with fewer resources. This solution has delivered a 25 percent decline in hospital readmissions, while also providing evidence-based care assessments that ensure staff are providing the same level of care to all residents.

Harmony Insights is a person-centered analytics solution that provides organizations with performance metrics to improve relationships with their acute care partners and win more referrals. With Harmony Insights, providers can capture and compile the information needed for real-time, data-driven conversations with acute-care partners. They can also gain more visibility into high-level metrics to find opportunities for improvement and have insight into nurse performance.

The third solution, PointClickCare Performance Insights, offers flexible reporting capabilities for key performance indicators and data to improve outcomes and reduce hospital readmissions. It allows providers to identify and understand performance and quality issues to reduce hospital readmissions. Performance Insights also identifies revenue drivers, helping providers meet their financial goals under PDPM.

All of these new products are integrated in PointClickCare’s electronic health record platform, allowing for better quality outcomes.

In addition to introducing these new products, PointClickCare is also working hard to help home care providers effectively prepare for the Patient Driven Groupings Model (PDGM) on January 1, 2020. With the implementation of PDGM, providers will need to obtain all clinical documentation in a timely manner, in order to bill and not interfere with cash flow.
