How to Achieve Success with PDGM

September 1, 2020 Stephanie Mancini

When the Patient-Driven Groupings Model (PDGM) was introduced earlier this year, Home Health agencies began the transition from a volume-based payment model to the new value-based payment system. This change has impacted every aspect of an agency and requires a complex planning and execution process in order to ensure that not only are all of your tasks being completed properly, but that your organization is also thriving in this new setting.

We understand that this is a big lift for agencies, and that with so many moving parts, it’s sometimes difficult to keep all stakeholders and their corresponding moving parts organized. To help you ensure that you are keeping all items in check, we have created a PDGM Toolkit. In the toolkit, you’ll find a  series of checklists for each major PDGM stakeholder group, including:

  • Administrators
  • Intake Coordinators
  • Case Managers
  • Billing Managers
  • Oasis Coders
  • Order Manager Tracker

Each checklist is specific to the role and helps the stakeholder to identify, plan for, and complete their individual tasks. In the checklist you can also take notes about each item so you can keep the rest of your agency in the loop. Working together, the checklists ensure that all tasks for the entire organization are completed in a timely and efficient manner. Because as you know, PDGM is an organization wide change affecting all operations and a cohesive collaboration on all fronts is vital to your success.

With this toolkit we aim to provide the strategies and organizational tactics required to ease your transition to PDGM and enable you to face the new quality-driven world of PDGM with confidence.
