Big News From Harmony: Looking Back & Pushing Ahead

The uncertainties of COVID-19 have tested our healthcare system as a whole. Health system and acute care leaders have had to find new ways to enable collaboration and coordination between teams inside their facilities and across their networks. Meanwhile, those at the front lines of post-acute care have faced the ultimate challenge of breaking down data barriers to better collaborate with their health system partners in the name of driving better outcomes and saving lives. Data regarding the status of patients, facilities, and networks – and the ability to share that information across the healthcare continuum – have never been more critical.

Care transitions, in particular, continue to be an area of focus for acute and post-acute care facilities alike – especially now when the dangers of disjointed transitions can increase the risk of possible COVID-19 infection, and strain our already overburdened ER departments.

In the last year, integrated care coordination has emerged as a new strategy to help manage post-acute patients and facilities. I’m proud of the work we’ve done to support leading hospital systems like Kaiser Permanente, TriHealth, and the University of Maryland Medical Center. These organizations embraced integrated care coordination to support case managers, preferred network managers, and their skilled nursing partners – and the results are significant:

  • TriHealth has improved confidence around care transitions by sharing patient info – like current medications – and clinical data with skilled nursing partners prior to a patient’s arrival. By making discharge orders and clinical data actionable as part of the clinician’s workflows, skilled nursing partners can begin delivering care immediately, improving the likelihood of a successful transition and reducing the risk of readmission in the first 72 hours.
  • Kaiser Permanente has strengthened the relationships with their skilled nursing providers by bringing together data about facilities and patients in one place. By understanding the drivers of facility performance and patient outcomes based on real-time source data, Kaiser Permanente can now create data-driven performance management conversations and build preventative plans with post-acute network partners.
  • The University of Maryland Medical Center is monitoring high-risk patients during their long-term post-acute stay by receiving live updates regarding changes in patient health. By synchronizing key information that drives clinical workflows and enabling collaborative care planning between acute and post-acute partners, the University of Maryland Medical Center is now able to take action when a patient starts to degrade.

While we’ve accomplished a great deal in the last year with the support of our customers, we’ve also seen the care landscape evolve drastically and radically. In response, Harmony has evolved as well.

New in 2020

The latest 2020 Harmony release gives case managers new insight into the status of their patients in post-acute settings with real-time chart access, allowing them to scale their caseloads. Plus, preferred network managers now have access to new tools that enable them to understand their network performance and engage in proactive, data-driven performance management conversations with their partners.

Here’s what’s new in Harmony:

  1. Understand network performance at a glance. Harmony makes it easier to access the types of information hospital systems are most commonly using to measure through network scorecards (census data, admissions, LOS, and more), organization categories, and CMS ratings.
  1. Enable meaningful, insight-driven performance conversations. Harmony offers new ways to visualize patient populations and understand what’s driving metrics, allowing hospital systems to begin reviewing performance from the last 90 days.
  1. Scale caseloads by bringing focus to the right patients. Harmony helps case managers shortlist the patients they should be focusing on by presenting the right information as fast as possible, helping them to decide if they need to take action and when.
  1. Manage patient infections at skilled nursing facilities. Harmony gives case managers and post-acute network managers a real-time view into infections at skilled nursing facilities – including tracking active COVID-19 patients.
